SplitForms Booking · Day spas
Spa booking software for several staff and treatments
SplitForms Booking is spa booking software for day spas. Clients book a treatment with a named therapist, or into one of a set number of spaces, answer intake questions for that treatment and can pay a Stripe deposit. Front desk and therapists get their own roles. It costs $100/year for 10,000 bookings, however many staff you add.
14-day free trial, no card. Booking is a paid add-on: $100/year per account.
The first decision
Book people, or book spaces
A treatment books in staff mode as soon as any staff member can perform it. With no staff, the page books in capacity mode: you set how many clients can book the same time.
| Staff mode | Capacity mode | |
|---|---|---|
| Client picks | A therapist or “Anyone available” | Just a time |
| Open when | Someone who does the treatment is working and free | Fewer bookings overlap than your spaces per time |
| Hours | Each person’s own week and time off | The spa’s opening hours |
| Good for | Massage and facials where skill or client preference matters | Fixed rooms or stations where anyone on shift can do the treatment |
Worked example
One Saturday, both ways
Open 10:00–18:00, start times every 30 minutes, 60-minute treatments and a 15-minute break after each booking. Both tables are computed by the code that runs the booking calendar.
Capacity mode: 3 spaces
Booked: two clients at 10:00, one at 11:00. The 14:00 start is cut to 1 space (a room is out of use).
| Start | Spaces left |
|---|---|
| 10:00 | Full |
| 10:30 | Full |
| 11:00 | Full |
| 11:30 | 2 |
| 12:00 | 2 |
| 12:30 | 3 |
| 13:00 | 3 |
| 13:30 | 3 |
| 14:00 | 1 |
| 14:30 | 3 |
| 15:00 | 3 |
10:00 is full even with one space unused: the 11:00 client would overlap a new 10:00 treatment once the break is counted. Every overlapping booking, break included, takes a space.
Staff mode: Ana, Ben, Chloe
Ana does massage and facials, Ben massage only, Chloe facials and works from 12:00. Booked: Ben: massage 10:00–11:00; Ana: 90-minute facial 12:00–13:30.
| Start | 60-minute massage | 60-minute facial |
|---|---|---|
| 10:00 | Ana | Ana |
| 11:00 | Full | Full |
| 12:00 | Ben | Chloe |
| 13:00 | Ben | Chloe |
| 14:00 | Ana, Ben | Chloe, Ana |
Ana’s facial at 12:00 also takes her out of massage, because one person can’t be in two rooms. Staff mode tracks people, not rooms, so make sure you have a room for every therapist on shift.
Running the spa
Treatments, intake and a team with roles
One break for every treatment
Each treatment has its own length and price, but the break after each booking is one setting for the whole page. If a body wrap needs longer to reset than a facial, add the extra minutes to the wrap’s length instead of raising the break for everything.
Intake per treatment
Limit each intake question to the treatments it matters for: pregnancy and pressure for massage, skin sensitivities for facials. Private answers are encrypted at rest. SplitForms is not HIPAA compliant and does not sign a BAA (security).
Front desk, managers, therapists
Front desk sees every booking, books clients in by phone and runs the waitlist. Managers also change settings, see reports and issue refunds. A therapist with the practitioner role sees only their own appointments.
Reports by treatment and therapist
See appointments, money collected and no-shows per treatment and per staff member, plus how much of your open time was booked. When someone cancels, the first person on the waitlist gets an email.
Deposits hold the slot for 30 minutes while the client pays on Stripe; see deposits and no-show fees. Email and text reminders go out before every treatment (20 free SMS credits to start).
Honest fit
When a full spa suite is the better choice
- You sell packages, prepaid series, memberships or gift cards: SplitForms has none of these.
- Guests book spa days with several treatments in one go: each booking is one treatment.
- You need rooms and therapists scheduled together: staff mode tracks people, capacity mode tracks spaces, not both.
- Couples treatments are common: one booking holds one staff member.
- You run retail, a point of sale, tips or commissions from the same system.

Setup
Set up spa booking in four steps
Choose staff mode or capacity mode
Add your therapists and estheticians as staff if clients book a person. Leave staff out and set “Spaces per time” if clients just book a treatment and a time.
Build the treatment menu
Add each treatment with its own length and price, and assign it to the staff who perform it. One break after each booking applies to every treatment.
Ask per-treatment intake questions
Start from the Client intake template, then limit questions to the treatments they matter for, such as pregnancy for massage or skin sensitivities for facials.
Give the front desk access and go live
Invite your front desk and therapists with their own roles, connect Stripe for deposits, turn on reminders and add the booking widget to your site.

What does spa booking software need to do for a day spa?
Let clients book a treatment with the right person, or into a free space, at an open time, take a deposit, collect intake answers and send reminders. SplitForms Booking does those; it does not do packages, memberships, gift cards or a point of sale.
Should my spa use staff mode or capacity mode?
Use staff mode when clients book a named therapist or each treatment needs a trained person: availability follows each person’s hours. Use capacity mode when clients book a time and any of a fixed number of spaces, such as treatment rooms; you set how many people can book the same time.
Does SplitForms support spa packages or memberships?
No. Each booking is one treatment with one staff member. There are no packages, prepaid series, memberships or gift cards, and a spa day with several treatments is booked as separate appointments.
Can I close one time or change how many people can book it?
Yes. Close a whole date or a single time, add a one-off extra time, or, on a page without staff (capacity mode), change the spaces at one time, for example down to one while a room is being cleaned or repaired.
How do deposits work for spa treatments?
Pick a deposit percent (25% by default) or full payment for the booking page. Each treatment’s deposit is that percent of its price, the slot is held for 30 minutes while the client pays on Stripe Checkout, and the money goes to your own Stripe account.
How much does it cost for a spa with several staff?
$100/year (USD) per account for 10,000 bookings a year, after a 14-day free trial. The price does not change with the number of staff. Dashboard logins use your forms plan’s team seats, and text reminders use SMS credits.
Is SplitForms HIPAA compliant for spa intake forms?
No. SplitForms is not HIPAA compliant and does not sign a BAA. Answers marked private are encrypted at rest; keep intake to what the treatment needs and keep medical records elsewhere.
