Personal trainer booking app for 1:1 and small-group sessions
SplitForms Booking is personal trainer scheduling software that runs in any phone browser. Clients book a 1:1 session or a space in a small group, answer your readiness questions and can pay upfront through Stripe. It costs $100/year per account for 10,000 bookings, after a 14-day free trial, with no app to install.
14-day free trial, no card. Then $100/year for 10,000 bookings.
Session types
1:1 training and small groups from one calendar
The Fitness & classes business type starts with a 60-minute “Class”, weekday hours 07:00–20:00, 60-minute start times, 1 hour’s minimum notice and a 12-hour cancellation cutoff. Change any of it.
1:1 sessions
Set spaces per time to 1 and each time can be booked by one client. Each session type has its own length (5 minutes to 12 hours), price and in-person, phone or video location.
Small-group sessions
Raise the spaces at a given date and time (up to 500) and several clients can book it until it’s full. Spaces are per time, not per service, so any booking at that time takes a space.
Several trainers
Add trainers as staff with their own hours, time off and Google Calendar, and clients pick a trainer or “Anyone available”. With staff, each trainer takes one client per time, so group spaces are off on that page.

Prices are examples; you set your own. “Due at booking” is calculated by the same rule the booking page uses, here with full payment switched on.
Sessions
| Service | Length | Where | Due at booking |
|---|---|---|---|
| Free intro call | 15 min | Phone | Free |
| 1:1 session | 60 min | In person | $80 |
| 30-minute express session | 30 min | In person | $45 |
| Online coaching session | 45 min | Video room | $60 |
| Small-group strength | 60 min | In person | $25 |
Booking rules
| Start times every | 30 minutes |
|---|---|
| Break after each booking | 15 minutes (reset the space, travel between gyms) |
| Spaces per time | 1, so ordinary times are 1:1 |
| Spaces changed | 6 at 09:00 on each Saturday you run the group |
| Minimum notice | 12 hours |
| Book up to | 60 days ahead |
| Payments | Full payment at booking |
The readiness questions apply only to the in-person and group sessions, so an intro call stays quick. Because spaces are per time, a 1:1 booked at 09:00 on a group Saturday takes one of the six spaces, so tell 1:1 clients to pick another time.
Before the first session
Readiness questions as plain booking fields
“Health check & waiver” template
- Date of birth
- Injuries, conditions or surgeries we should know about
- Has a doctor ever told you not to exercise?
- Emergency contact name and phone
- Experience level
Plus a risk acknowledgement clients must tick to book. Edit any of it.
Add PAR-Q style questions
- Has a doctor ever said you should only do exercise they recommend? (yes / no)
- Do you feel chest pain or lose balance from dizziness when active? (yes / no)
- Are you taking medication that affects exercise? (yes / no, private)
- Anything else your trainer should know? (paragraph, private)
These are ordinary form fields: a “yes” doesn’t stop the booking, it’s there for you to read and follow up. Private answers are encrypted at rest. splitforms doesn’t give medical advice, isn’t HIPAA compliant and signs no BAA (security).
Payments & reminders
Paid before the session, reminded the day before
Full payment or a deposit
Clients pay on Stripe Checkout at booking and the money lands in your own Stripe account. No card is saved, so a late-cancel fee has to come from a deposit or a payment link you send. See no-show fees and deposits.
Reminders by email and text
Reminder emails and opt-in texts go out before each session (20 free SMS credits to start), with a link to reschedule or cancel inside your cutoff.
Your regulars, by hand
There are no recurring bookings, so book a regular client’s next sessions yourself from the dashboard. Online notice rules don’t apply when you add a booking.
Honest fit
When splitforms is not the right fit
SplitForms Booking covers booking, payment at booking, readiness questions, reminders and trainer calendars. It has no recurring bookings, session packages, memberships or class passes, no workout programming or progress tracking, and no native app. Each account has one booking page, so a gym that needs per-trainer calendars and group classes side by side is better served by a gym management platform. For a trainer who wants clients to book and pay online, this is the lighter, flat-priced option.
Only need an enquiry form with no calendar? Use the free personal trainer contact form template.
Setup
Set up personal training bookings in four steps
Pick “Fitness & classes” and add sessions
Create a booking page with the Fitness & classes business type, then edit the starter class into your 1:1, express, online and group sessions.
Choose 1:1 or group spaces
Leave spaces per time at 1 for 1:1 training. For a small group, raise the spaces at that date and time so several clients can book it.
Add readiness questions
Start from the Health check & waiver template and add your own yes/no questions in a PAR-Q style. Mark health answers as private.
Take payment and share the link
Connect Stripe for full payment or a deposit, turn on reminders, and put the booking link in your bio, website and client messages.

Frequently
asked.
Anything missing? Email us and we’ll answer.
Is there a personal trainer booking app for iPhone or Android?
No native app. SplitForms Booking is a booking page and website widget that clients open in their phone's browser, so there is nothing to install. You run your calendar from the dashboard in your own browser.
How much does SplitForms personal trainer scheduling software cost?
$100/year (USD) per account for 10,000 bookings a year, after a 14-day free trial with no card. Text reminders use SMS credits, bought separately.
Can I run small-group sessions as well as 1:1 training?
Yes, on a page without staff. Spaces per time sets how many clients can book the same time (1 for 1:1), and you can raise the spaces for a specific date and time to run a group. Spaces count every booking at that time, whatever the service.
Can clients book a recurring weekly session?
Not automatically. There are no recurring bookings, packages or session credits. Clients book each session (up to two years ahead), and you can add bookings for regular clients yourself from the dashboard.
Can I ask PAR-Q style health questions before a session?
Yes, as ordinary booking questions. The “Health check & waiver” template asks date of birth, injuries, whether a doctor advised against exercise, an emergency contact and experience level, plus a risk acknowledgement clients must tick. splitforms doesn't screen or assess answers, isn't HIPAA compliant and signs no BAA.
Can clients pay for sessions when they book?
Yes. Ask for full payment or a deposit. The client pays on Stripe Checkout while the slot is held for 30 minutes, and the money goes to your own Stripe account. Free intro calls priced at $0 book without paying.
Does it work for online personal training?
Yes. Set a service to video and it gets its own meeting link, a new video room or a Google Meet link, and the join link is included in the reminders.
