File Upload Form Template

Email attachments cap at 25MB and clog inboxes. A file-upload form accepts bigger files, multiple files, and lands them in one place — your dashboard, Slack, or cloud storage — with structured metadata.

No backend code

No server, API route, or SDK — your File Upload Form posts straight to one endpoint.

Straight to your inbox

Every submission is emailed to you and saved to a searchable dashboard — spam filtered before it reaches you.

Copy-paste ready

It's plain HTML you own — paste the File Upload Form snippet, swap in your access key, and ship.

HTML by default — the same field set also renders as React, Next.js, PHP, and cURL in your dashboard. Every variant posts to the same /api/submit endpoint. Replace YOUR_ACCESS_KEY with the key from your dashboard; that's the only edit.

How to add it

How to add a File Upload Form to your site.

To add a File Upload Form to your site you need three things: a free splitforms access key, the HTML snippet above, and your key pasted into it. No backend, database, or CAPTCHA library — the form posts to one URL and every submission lands in your inbox and dashboard.

Add the drag-and-drop zone

Copy the file-upload template — a labelled dropzone with file-type filtering and a removable-file list. The form posts to splitforms as multipart/form-data, same as any other submission.

Capture context alongside the file

Add a description field, a category dropdown, and the submitter's name and email. Files without context are noise — 'design brief' or 'week 3 report' alongside the attachment turns the upload into actionable information.

Route to your tools

Webhook the submission (with signed download URLs for each file) to Slack, Google Drive, Notion, or your project tool. The file lives in storage; the notification lives wherever your team already works.

What's included

What's in this File Upload Form template?

Email is a terrible file-transfer system — size caps, blocked attachments, no metadata, lost in threads. A dedicated upload form solves three problems at once. It accepts larger files than email (up to 5MB per file, 5 files per submission), it captures context alongside the file (who uploaded, why, related reference), and it routes the package wherever you want — Slack notification, Google Drive folder, Notion database row. Use cases: clients sending design briefs, applicants uploading portfolios, customers attaching bug screenshots, patients sending intake paperwork. 5MB per file, 5 files per submission · webhooks to Slack / Drive / Notion with signed URLs.

  • Basic contact form — 4 ready-made fields
  • HTML, JS, React, PHP, and cURL outputs
  • One POST endpoint, no SDK — honeypot and classifier, no CAPTCHA

Where submissions go

Where do your File Upload Form submissions go?

Reply-to is wired to the visitor's email, so one tap on Reply answers them directly — while the built-in honeypot and spam classifier filter out bots before they reach you. Every submission is emailed to your inbox and can forward to a webhook or Slack the moment a visitor hits send.

  • Reply-to wired to the visitor's email — reply straight from your inbox
  • Honeypot and spam classifier filter bots, no CAPTCHA friction

Questions

File Upload Form questions.

View all FAQs
What file types can the form accept?

Anything you whitelist in the input's accept attribute — PDFs, images, Word and Google docs, spreadsheets, ZIP archives, audio, video. Server-side validation enforces both extension and MIME type. Executable types (.exe, .bat, .sh) are blocked by default for security.

How big can the files be?

Up to 5 files per submission with a 5 MB cap per file. That covers most document, image, and short-video uploads. For larger files (raw video, design source files), capture a Dropbox / Google Drive / WeTransfer link in a text field instead of asking for direct upload.

Where do the uploaded files live?

Files are stored in Supabase Storage after you connect the Storage integration in your dashboard. Each upload gets signed download URLs that only the form owner can generate. Files are deleted when you delete the submission.

Can the form forward files to Slack or Google Drive?

Yes — the webhook payload includes signed download URLs for each file. Slack webhooks render the URL as a clickable link; Google Drive and Dropbox integrations can fetch the file from the URL on receipt. The file itself stays in splitforms storage; downstream tools link to it.

Is file upload safe for sensitive documents?

Files are encrypted in transit and at rest, and download URLs are signed against your account. For genuinely sensitive material (medical records, financial statements, government IDs), use a dedicated secure-upload portal with the right compliance — not a public web form. Treat the splitforms upload form like email: appropriate for business documents, not for regulated data.

Does the dropzone work on mobile?

Yes — the drag-and-drop zone falls back to a tap-to-select file picker on touch devices. Mobile users can also photograph and upload directly from the camera via the file input. Test on iOS Safari and Android Chrome; both work with native file pickers.
