Employee Evaluation Form Template

Updated · plan facts checked against /pricing

An employee evaluation form records how a manager rated someone's work over a set period, what goals come next, and that both people saw the result. This free HTML template has a labelled 1-5 scale for six competencies, an overall rating, three goals, employee comments, and typed-name acknowledgements from reviewer and employee.

Field by field

What each field collects.

scale key (paragraph)
The meaning of each point is written once, above the ratings, so every reviewer reads 3 as 'meets expectations'. Change the wording if your company uses its own scale, but keep it on the form.
rating_quality ... rating_reliability (radios, required)
One field per competency, so each becomes its own column in the dashboard and in CSV exports on paid plans. Swap the six legends for the competencies that matter in the role; keep the rating_ prefix so the columns sort together.
overall_rating (select, required)
Picked by the reviewer, not averaged by the form. An average of six scores can hide the one area that decides the review, so the reviewer states the overall judgement on its own.
period_start and period_end (dates, required)
Fixes what time span the scores cover and reminds the reviewer to rate the whole period, not only the most recent weeks.
strengths and improvements (required)
Both labels ask for examples. A rating with no example is hard to act on and hard to explain later.
goal_1 to goal_3
Only the first goal is required. Each label asks for what, how it will be measured and by when, which turns 'communicate better' into something the next review can check.
email (reviewer)
Named email so it becomes the Reply-To on the notification. HR can answer the reviewer straight from the inbox.
employee_ack + employee_typed_name
A required checkbox and a typed name, completed by the employee at the end of the review meeting. The checkbox text says it confirms receipt, not agreement. splitforms has no draw-your-signature field.
form_version (hidden)
Change it when you change the competencies or the scale, so you can tell which version of the form each evaluation used.

How to add it

How to add this form to your site.

To add a Employee Evaluation Form to your site you need three things: a free splitforms access key, the HTML snippet above, and your key pasted into it. No backend, database, or CAPTCHA library — the form posts to one URL and every submission lands in your inbox and dashboard.

Set your competencies and scale

Replace the six competency legends with the ones that fit the role, adjust the scale wording if your company has its own, set form_version, and swap in your access key.

Fill it in during the review meeting

The reviewer completes the ratings, comments and goals, then hands the device to the employee for their comments, acknowledgement checkbox and typed name before submitting.

Keep the record

Each evaluation is emailed to you on every plan and saved in the dashboard with the time it was received. On paid plans, export the CSV to compare ratings across a team or across review cycles.

What's included

What's included.

Most evaluation forms fail in the same two places. The first is the scale: a row of unlabelled numbers lets one manager's 3 mean 'fine' and another's mean 'struggling', so scores can't be compared across a team. This template prints the meaning of every point once, above the ratings, and rates each competency as its own field so each one becomes its own column. The second is the record of what happened afterwards. The form ends with the employee's own comments and two typed-name acknowledgements, and the employee's checkbox says that typing a name confirms they received the review, not that they agree with it. Fill it in together at the end of the review meeting and the submission is the complete record, stamped with the time splitforms received it. Labelled 1-5 ratings per competency, measurable goals, and a receipt-not-agreement acknowledgement with typed names.

  • Employee evaluation form: 26 fields (employee, reviewer, review type and period, six 1-5 competency ratings, overall rating, strengths, improvements, three goals, next review date, employee comments, reviewer and employee acknowledgement with typed names) plus a hidden form version
  • Plain HTML with labelled fieldsets — paste it into any site
  • One POST endpoint, no SDK — honeypot and spam scoring, no CAPTCHA

Questions

Frequently asked questions.

View all FAQs
What should an employee evaluation form include?

The employee and reviewer names, the review period, a labelled rating scale, a rating for each competency you care about, an overall rating, written strengths and areas to improve with examples, goals for the next period, space for the employee's comments, and an acknowledgement from both people. This template has all of them.

Which rating scale should I use?

A 5-point scale with every point described is the common choice: it leaves room between 'meets' and 'exceeds' without asking for false precision. Whatever you choose, describe each point on the form and use the same scale for everyone you review.

Does the employee have to agree with the evaluation?

This template asks the employee to acknowledge receipt, not agreement, and gives them a comments box to record any disagreement. Whether a typed name and checkbox is enough for your records depends on your company policy and local employment law, so check with HR or a lawyer. splitforms has no draw-your-signature field.

Can I use the same form for self-evaluations?

Yes. Copy the form, change the subject to 'Self-evaluation', remove the reviewer acknowledgement, and send the link to employees before the review meeting. Comparing their scores with the manager's ratings for the same period is a quick way to find where to focus the conversation.

Who can see the evaluations?

Each one is emailed to your notification address and saved in your splitforms dashboard, so anyone with access to that account or inbox can read it. Paid plans include 3 team seats; invite only the people who should see personnel records.

Is there a printable version?

This is an online form. You can print the blank page from your browser for a paper review, but only forms submitted online reach your inbox and dashboard.

Is the employee evaluation form free?

Each evaluation is one submission, and the Free plan covers 500 in total with no card. CSV export, for comparing ratings across reviews, is on paid plans; Pro is $5 per month.
