Capture product identifiers
Required: product model, serial number, purchase date, retailer. The serial number is the key — it ties this registration to a specific unit on your production line for warranty validation and recall lookup.
Form template · Detailed contact form
Product registration isn't paperwork — it's how you reach owners for recalls, upsell extended warranties, and validate service claims. The form captures the fields your service team needs from day one.
Every template is free and fully editable in the drag-drop builder — swap fields, colors, and the button label before you publish.
No server, API route, or SDK — your Warranty Registration Form posts straight to one endpoint.
Every submission is emailed to you and saved to a searchable dashboard — spam filtered before it reaches you.
It's plain HTML you own — paste the Warranty Registration Form snippet, swap in your access key, and ship.
Copy and ship
HTML by default — the same field set also renders as React, Next.js, PHP, and cURL in your dashboard. Every variant posts to the same /api/submit endpoint. Replace YOUR_ACCESS_KEY with the key from your dashboard; that's the only edit.

</p>How to add it
To add a Warranty Registration Form to your site you need three things: a free splitforms access key, the HTML snippet above, and your key pasted into it. No backend, database, or CAPTCHA library — the form posts to one URL and every submission lands in your inbox and dashboard.
Required: product model, serial number, purchase date, retailer. The serial number is the key — it ties this registration to a specific unit on your production line for warranty validation and recall lookup.
Owner name, email, phone, installation date (if installed by a pro), installer name for HVAC / appliance / electronics that need setup. Installer info matters when warranty terms require authorised installation.
Webhook submissions into your CRM or product database — Salesforce, HubSpot, a custom table — keyed on serial number. Future service calls and recall notifications look up the owner by serial.
What's included
Manufacturers who skip product registration pay for it three ways: they can't notify owners about safety recalls (a legal liability in most jurisdictions), they can't validate in-warranty service claims without a receipt hunt, and they lose the post-purchase marketing channel for accessories and extended warranties. The form captures model, serial number, purchase date, retailer, and owner contact — five fields that turn a one-time sale into a known customer relationship. Recalls become a one-email broadcast instead of a regulatory crisis. Serial number is the key — turns a one-time sale into a recall-able, serviceable customer relationship.

Where submissions go
Reply-to is wired to the visitor's email, so one tap on Reply answers them directly — while the built-in honeypot and spam classifier filter out bots before they reach you. Every submission is emailed to your inbox and can forward to a webhook or Slack the moment a visitor hits send.

Testimonials
40 quotes on record — from indie hacks to agency migrations.
“I replaced a Lambda + DynamoDB + SES contact form with six lines of HTML. It took eleven minutes, and the dashboard is better than what I was going to build.”
“We migrated 14 client sites off Formspree in a single weekend. The price is a third of what we paid, the API is more honest, and the spam filter actually works.”
“The webhook payload is signed, idempotent, and well-shaped. It reads like code from a competent team, not a CRUD app held together with duct tape.”
“I stopped reaching for Typeform on small marketing sites. splitforms covers 90% of the use case at none of the bloat.”
“I onboarded our whole agency in an afternoon. The MCP integration meant Cursor literally dropped the form straight into our client repos for us.”
“The free plan gave me 500 submissions before I paid a cent, and Pro is five dollars a month. I've spent more on coffee deciding which backend to use.”
“Spam went from forty junk entries a day to zero, with no reCAPTCHA puzzle ruining the form. The honeypot and time-trap just quietly do their job.”
“Point the form action at one endpoint and you're done. No SDK, no client library, no build step. This is how a form backend should feel.”
“Leads land in Slack the second someone submits, and a copy goes to Google Sheets for the sales team. I wired both up in under ten minutes.”
“I run a static Hugo site on a five-dollar VPS. splitforms gave it a real contact form without me standing up a single server.”
Questions
Most jurisdictions (US Magnuson-Moss, EU consumer law) require you to honour the warranty with proof of purchase, not just registration. You can encourage registration with extended-coverage perks, but you can't deny a valid claim because the customer didn't fill out the card.
Capture the base registration, then offer an extended warranty in the auto-responder with a Stripe or Shopify checkout link. Time it for 30-60 days before the standard warranty expires — that's when customers are most receptive to paying for extension.
Yes — that's one of the main reasons to collect registrations. Filter the database by model and serial range, then bulk-email affected owners through your ESP. Regulators in the US (CPSC), EU (Safety Gate), and elsewhere require reasonable effort to notify owners of safety recalls.
Free-text with a pattern hint ('usually 8-12 characters on the rating plate'). Dropdowns only work for very small product lines. Free-text plus server-side validation against your serial format catches typos without frustrating customers who can't find their model in a list.
Add an unbundled checkbox: 'I'd like to receive product updates, tips, and offers.' Pre-ticked boxes are non-compliant in the EU and UK — consent has to be affirmative. Required fields cover the warranty itself; marketing consent is optional and gates downstream email.
