Employment Application Form Template

Most job sites route every applicant through a 30-question ATS flow. For high-volume roles — retail, warehouse, hospitality — a one-page application form converts at 3-5x the rate and lands in your inbox.

HTML by default — the same field set also renders as React, Next.js, PHP, and cURL in your dashboard. Every variant posts to the same /api/submit endpoint. Replace YOUR_ACCESS_KEY with the key from your dashboard; that's the only edit.

How to add it

How to add a Employment Application Form to your site.

To add a Employment Application Form to your site you need three things: a free splitforms access key, the HTML snippet above, and your key pasted into it. No backend, database, or CAPTCHA library — the form posts to one URL and every submission lands in your inbox and dashboard.

Capture the legally required fields

Required: full name, email, phone, position applying for, right-to-work confirmation, availability (full-time / part-time / specific shifts). Add EEOC self-identification fields only if you're a covered employer — keep them on a separate page from the application itself.

Ask for work history or resume

Optional resume upload (Pro) plus a short work-history textarea for applicants without a polished resume. Frontline and entry-level applicants often have work history but no PDF — don't lock them out by requiring an upload.

Route to the hiring manager

Webhook applications into your ATS (Greenhouse, Lever, Workable) or a shared Notion / Airtable pipeline for smaller teams. Tag by position and source so the right recruiter sees the right candidates.

What's included

What's in this Employment Application Form template?

Enterprise ATS (Workday, Taleo, UKG) optimise for compliance and lose applicants at every step — industry data shows 60-80% abandonment on multi-page application flows. For high-volume hiring (retail, hospitality, warehouse, frontline), a single-page application form with the legally required fields converts dramatically higher. Capture name, contact, position, eligibility, availability, and an optional resume upload. Route to the hiring manager or a simple Notion / Airtable pipeline. As you grow into regulated hiring (federal contractors, finance), graduate to a full ATS — the form stays useful as the front door. Single-page application converts 3-5x better than enterprise ATS flows for high-volume hiring.

Questions

Employment Application Form questions.

View all FAQs
Do we need EEOC self-identification fields?

Only if you're covered by EEOC reporting requirements (15+ employees) or are a federal contractor (OFCCP). If covered, the self-ID (race, gender, veteran, disability) goes on a separate page with explicit 'voluntary, refusal does not affect hiring' language. Consult your employment counsel on the exact wording and storage rules.

Should we require a cover letter?

For senior or specialised roles, yes — quality of applicant goes up. For high-volume frontline hiring, no — required cover letters drop applications 60-80% and add little signal at that level. Tune by role; one form can branch on the position field.

How do we avoid staffing-agency spam?

Honeypot plus splitforms' classifier catches most. For persistent agency submissions, add a 'are you applying directly or representing an agency?' radio — direct applications get the role-specific reply; agency submissions get a templated 'we don't accept third-party candidates' response.

Can this integrate with Greenhouse, Lever, Workable?

Yes — webhook the JSON to the ATS via their candidate-API or a Zapier connector. The application lands as a candidate with position, contact info, and resume attached. As you scale into formal ATS-driven hiring, the form stays as the front door — same UX, different backend.

What about background checks and I-9 verification?

Not on the application form. Capture interest and basics here; run background checks (Checkr, Certn) and I-9 verification (E-Verify) after a conditional offer. Embedding legal verification on the public application form creates compliance risk and drops completion.

Should we ask for salary expectations on the form?

Optional, and frame it as a range rather than a single number. Some jurisdictions (NY, CA, CO, WA) ban salary-history questions but allow expectations. A range dropdown filters obvious mismatches without the legal risk of asking what they currently make.
